When you first log in to MyChart, you can provide an e-mail address to which alerts will be sent. These alerts will not contain any specific information from your MyChart account; rather they will notify you that you have updated information in MyChart to review (such as new test results, a new message from your doctorís office or an appointment reminder). Yes, this is called ďproxyĒ access. For a child under 12, this allows a parent or legal guardian to log into their personal MyChart account, and then view information regarding their child. Per State and Federal guidelines, after the child becomes 12 the parent will still have access to appointments and be able to send messages but will have limited access to the childís medical record information. Once the child turns 18, the parent can get access to the patientís medical record information with the patientís authorization. It is also possible to request access to another adultís health record if you help manage that adultís medical care. For more information please inquire about our Proxy Access FAQ Form. To receive an access code for your own health record information, complete the MyChart Sign-Up Form available at your doctorís office or obtain the form from our sign-up page. To receive an access code to view a family memberís health information, complete the Proxy Form. In addition, if you turn in the forms at your doctorís office, your access code will typically be given to you immediately. You may also mail or fax the Sign-Up and Proxy Forms to our Health Information Management (HIM) Department using the contact information that can be found on the forms. If you need additional information than what is available through MyChart, you may complete an Authorization for Release of Health Information Form and turn it in at your doctorís office or mail or fax it to the address/fax number on the form.Return to Top

DuPage Medical Group and Edward are jointly providing MyChart to improve the coordination of and access to care for patients. If you see both DuPage Medical Group and Edward physicians, you will see your medical information from both physician groups and Edward Hospital visits in MyChart. You will also be able connect online with both physician groups through your one MyChart account. And, you will conveniently have only one username and password for MyChart!
If you see a physician/s at one group but not the other, your MyChart account will reflect your medical information only from that provider and will not be connected to or accessible by other physician offices. For instance, if your primary care is at DuPage Medical Group, and you donít see any physician at Edward Medical Group, only DMG staff will have access to your records.Return to Top
